What is DA Form 2404?
DA Form 2404 is the Army's Equipment Inspection and Maintenance Worksheet used to record faults found during PMCS. It documents the fault description, status, corrective action, and parts required for ground vehicles.
What is the difference between DA 2404 and DA 5988?
DA Form 2404 is the older paper-based fault recording worksheet used primarily for ground vehicles. DA Form 5988-E is the newer electronic version used in SAMS-E. Both serve the same purpose — documenting equipment faults and maintenance actions.
