Common Questions
What is the difference between DA 5988 and DA 5988-E?
Same form, different format. DA Form 5988 is the paper version; DA Form 5988-E is the electronic/fillable version. Both are the Equipment Inspection and Maintenance Worksheet and are used interchangeably in the field. Most soldiers just call it "the 5988."

What does Not FMC mean on a 5988?
Not FMC means Not Fully Mission Capable — the vehicle has a fault that affects its ability to perform its assigned mission. Not FMC items must be documented on the 5988 and reported to the maintenance chain.
Do I need a 5988 for every PMCS interval?
Yes. A separate DA Form 5988 is typically used for each PMCS interval (Before, During, After, Weekly, Monthly). Each interval has its own checklist items per the vehicle's Technical Manual.
